The Preoperative CT-Scan Can Help to Predict Postoperative Complications after Pancreatoduodenectomy
نویسندگان
چکیده
After pancreatoduodenectomy, complication rates are up to 40%. To predict the risk of developing postoperative pancreatic fistula or severe complications, various factors were evaluated. 110 consecutive patients undergoing pancreatoduodenectomy at our institute between January 2012 and September 2014 with complete CT scan were retrospectively identified. Pre-, per-, and postoperative patients and pathological information were gathered. The CT-scans were analysed for the diameter of the pancreatic duct, attenuation of the pancreas, and the visceral fat area. All data was statistically analysed for predicting POPF and severe complications by univariate and multivariate logistic regression analyses. The POPF rate was 18%. The VFA measured at umbilicus (OR 1.01; 95% CI = 1.00-1.02; P = 0.011) was an independent predictor for POPF. The severe complications rate was 33%. Independent predictors were BMI (OR 1.24; 95% CI = 1.10-1.42; P = 0.001), ASA class III (OR 17.10; 95% CI = 1.60-182.88; P = 0.019), and mean HU (OR 0.98; 95% CI = 0.96-1.00; P = 0.024). In conclusion, VFA measured at the umbilicus seems to be the best predictor for POPF. BMI, ASA III, and the mean HU of the pancreatic body are independent predictors for severe complications following PD.

Evaluation of the Relationship between Mandibular Impacted Wisdom Teeth and Mandibular Canal in Preoperative Panoramic and Postoperative Complication
Background & Aims: Before the surgery of lower impacted third molars, it is essential to evaluate the relationship of the roots to mandibular canal using radiographies in order to avoid postoperative complications. The purpose of this study was to evaluate the reliability of panoramic radiographs in assessing this relationship. Methods: This was a prospective study. The study population consist...
متن کاملPreoperative Computed Tomography to Predict and Stratify the Risk of Severe Pancreatic Fistula After Pancreatoduodenectomy
The aim of this article is to assess whether measures of abdominal fat distribution, visceral density, and antropometric parameters obtained from computed tomography (CT) may predict postoperative pancreatic fistula (POPF) occurrence. We analyzed 117 patients who underwent pancreatoduodenectomy (PD) and had a preoperative CT scan as staging in our center. CT images were processed to obtain measu...
متن کاملReliability of Postoperative Radiographies in Ankle Fractures
Background: The accuracy of reduction of ankle fractures using postoperative plain radiographies (x-ray) remainswidely controversial. Some authors have demonstrated that postoperative computed tomography (CT) scan can beuseful for these patients. In current study, the efficacy of x-rays after fixation of ankle fractures was investigated basedon the CT scan findings.Metho...
متن کاملThe possum scoring system and complete blood count in the prediction of complications after pancreato-duodenal area resections.
UNLABELLED The introduction of markers which help in the identification of patients prone to suffer from postoperative complications enables to recognize them more easily and thus, treat them more effectively. THE AIM OF THE STUDY was to evaluate complete blood count indicators, as well as preoperative results obtained on the basis of the POSSUM and P-POSSUM scoring systems, considering the pre...
متن کاملPreoperative coronary calcium score is predictive of early postoperative cardiovascular complications in liver transplant recipients.
BACKGROUND Coronary computed tomographic angiography (coronary CT) is a non-invasive test for diagnosis of cardiac function. Coronary calcium scores determined by coronary CT are associated with cardiovascular risk factors. However, no studies have investigated the association between coronary calcium scores and cardiovascular complications after liver transplantation (LT). We therefore evaluat...
